HIV/AIDS Supplemental Funding (How do I open a ".zip" file?)
The application portion itself consists of two files:
  1. HIV AIDS supplemental fund_application.doc
  2. HIV AIDS Supplemental Fund_Budget.xls
The 2nd portion are supporting documents that PC requires the PCV to fill out and submit once their application has been approved:
  1. HIV AIDS Supplemental Fund_Project_Agreement.doc
  2. HIV AIDS Supplemental Fund-BANK_INFORMATION_FORM.doc
  3. HIV AIDS Supplemental Fund_Consent_for_Community_form.doc
  4. HIV AIDS Supplemental Fund_Consent_for_Volunteer_form.doc
The 3rd portion is for use during the project and when the project is completed:
  1. HIV AIDS Supplemental Fund_Project_Activity_Log.doc
  2. HIV AIDS Supplemental Fund_RECEIPT_FORM-EnglishThai.doc
  3. Sample_Receipt_for_HIV AIDS Supplemental Fund.doc (for reference)
  4. HIV AIDS Supplemental Fund_Project_Completion_Report_Form.doc

Thanks, this page has proved extremely helpful. Maybe someone can make a page specifically for requesting HIV-AIDS Supplemental Funding. I'd love a step-by-step explanation written by someone who has been through the process already.
